We support young people aged up to 25 years old, who are worried about their drug or alcohol use or who are at risk of lapsing/relapsing and would like some support around their use to improve their health and wellbeing WHAT DO WE DO? • Management of lapse and prevention of relapse. • Provide education and information sessions. • Brief intervention • Individual counselling • Group counselling • Advocacy and support HOW TO ACCESS OUR SERVICE Self-Referral: You can phone 1300 114 397 during office hours to speak to one of our Intake and Assessment Officers who will ask you some questions to help you determine which of our services would best meet your needs and arrange your first appointment at a location most convenient to you. Service Providers: We welcome referrals from Schools, Health Practitioners and other Service Providers and endeavour to minimise duplication of information gathering as much as possible. To refer you can phone 1300 114 397 or complete and email the attached referral form to MHFW@anglicaresq.org.au LOCATIONS: We offer our service in a number of community locations as well as Anglicare sites including: Roma, St George, Charleville, Cunnumulla, Quilpie, Thargomindah, Augathella, Yuleba, Injune, Wallumbilla, Surat, Ipswich, Riverview, Booval, Goodna, Rosewood, Gatton, Laidley, Lowood, Esk, Toogoolawah, Kilcoy.
